How to Submit a Claim

  1. Purchase your Lullaby Logic plan and save your receipt. Your Stripe receipt will be emailed to you immediately after purchase and will describe the service as "Sleep Health Coaching."
  2. Log into your FSA or HSA provider's portal and navigate to the reimbursement or claims section.
  3. Submit your receipt along with a description of the service. Use the term "Sleep Health Coaching" when describing the expense.
  4. If your plan requires a Letter of Medical Necessity (LMN), ask your pediatrician or family doctor to write one describing the medical need for sleep intervention for your child.
  5. Your plan administrator will review and approve or deny the claim based on your specific plan terms.

What to Tell Your Plan Administrator
